Medical Answering Service Cost in 2026 — A Practical Breakdown

Most medical answering service quotes are designed to be hard to compare. Here’s what the major vendors actually charge in 2026, what hidden fees to watch for, and how to figure out which pricing model fits your practice.

How medical answering service pricing works

There are three pricing shapes you’ll see:

Per-minute: e.g. Specialty Answering Service at $1.09–$1.54/minute, Posh at $1.90–$2.30/minute, Ruby at $3.45–$5/minute. Bundle a block of minutes per month, pay overage rates above.

Per-call: e.g. Smith.ai Virtual Receptionists at $6.69–$9.75/call. Bundle a number of calls per month.

Per-provider: e.g. DeskMD at $299–$449 per licensed provider per month, no minute or call caps. Smith.ai AI Receptionist also has per-provider tier at lower volumes.

Real published prices from six vendors (2026-05-03 pull)

Specialty Answering Service: $44/mo (economy) up to $10,599/mo (10K minutes). $1.09–$1.54/minute. HIPAA-compliant.

Posh: $65/mo entry up to $1,900/mo for 1,000 minutes. $1.90–$2.30/minute. HIPAA status unclear on website.

Ruby Receptionists: $250/mo for 50 minutes up to $1,725/mo for 500 minutes. ~$3.45–$5/minute. HIPAA-compliant.

Smith.ai Virtual Receptionists (live human): $292.50/mo for 30 calls up to $1,170/mo for 175 calls. $6.69–$9.75/call. Not HIPAA-compliant per Smith.ai’s own page.

Smith.ai AI Receptionist: ~$95/mo for ~60 calls. HIPAA not advertised.

DeskMD: $299/provider/mo Standard, $449/provider/mo Pro. Flat. No call or minute caps. HIPAA + signed BAA.

Hidden fees to watch for

Watch for these line items that don’t make the published price page:

Bilingual surcharge: many human services charge 25-50% extra for non-English calls.

Holiday rates: 1.5x or 2x base rates on holidays.

Setup fees: $200-$500 one-time, sometimes hidden as a “configuration” or “script-build” charge.

BAA fee: some general-business services charge $50-$100/month extra to bring HIPAA into scope.

Per-message dispatch: $0.50-$2.00 per SMS, separate from the call.

Call routing fee: warm transfers sometimes charge per-transfer.

DeskMD has none of these. Standard and Pro are flat per-provider per month, with $0 setup, BAA included, and SMS + email dispatch included.

What 5 different practices actually pay

Solo dental practice, 200 calls/month: Smith.ai Virtual at ~$700/mo for 100 calls. Posh at ~$215/mo for 100 minutes (assumes ~1 min/call). DeskMD at $299/mo (single provider).

3-physician primary care, 600 calls/month: Ruby at ~$720/mo for 200 minutes. Smith.ai Virtual at ~$1,170/mo for 175 calls. DeskMD at $897/mo (3 providers).

5-DVM veterinary clinic, 1,200 calls/month: Specialty Answering Service at ~$2,000/mo for 1,500 minutes. DeskMD custom plan (5+ providers, contact sales).

2-dentist practice, 400 calls/month: Posh at ~$700/mo for 350 minutes. DeskMD at $598/mo (2 providers).

Cardiology specialty, 250 calls/month with emergency triage: Ruby Cardiology Tier at ~$1,000/mo (base + emergency premium). DeskMD Pro at $449/mo (1 cardiologist).

When per-call pricing actually wins

Per-call pricing (Smith.ai, AnswerConnect) wins for genuinely low call volumes. The breakeven for a small practice is roughly 30 calls/provider/month at Smith.ai entry pricing — below that, paying $292/mo for 30 calls beats $299/mo flat.

The trap is that most practices think their call volume is low when it isn’t. Pull a month of inbound call logs from your phone provider before you commit to a per-call vendor. Practices regularly underestimate volume by 30-50% because they don’t see lunch-hour, after-hours, and weekend calls in their daily experience — those are the calls that go to voicemail.

A 3-provider practice modeled three ways

Worked example for a 3-provider primary care practice taking ~600 inbound calls/month (~200 per provider):

DeskMD Pro: 3 providers x $449 = $1,347/mo flat. Includes multilingual answering, advanced analytics, custom escalation rules, 1,825-day retention. No setup, no per-call charges, no holiday surcharge.

Smith.ai Virtual Receptionists ($1,170 plan, 175 calls): 600 calls means buying the top tier (175 calls) and paying overage on the remaining 425 calls. At ~$7-9/call overage, that’s $4,000-5,000/mo. Smith.ai also says this product is not HIPAA-compliant per their own page.

Ruby Receptionists ($720/mo, 200 minutes): Assuming 1.5 min average per call, 600 calls = 900 minutes. The 500-minute tier is $1,725/mo plus per-minute overage. Realistic monthly: $2,000-2,500/mo with overage.

For this profile, DeskMD is roughly half the cost of equivalent live-human service.

What to ask any answering service before signing

1. Sample BAA today. If they hesitate, walk.

2. What’s the average bill for a 3-provider practice doing 600 calls/month? If the answer is “depends,” ask for the spreadsheet they used to model it.

3. What’s the bill if call volume doubles? Per-provider scales linearly; per-call and per-minute can blow up.

4. Holiday + weekend rates? Get them in writing.

5. Bilingual surcharge? Get the surcharge percentage in writing, or confirmation it’s included.

6. Setup + onboarding? $0 should be the floor in 2026.

7. What happens to my data if I cancel? Recordings, transcripts, audit logs — what’s the retention or deletion process?

How to choose

Per-call pricing wins for low volume (under ~30 calls/month per provider). Per-provider pricing wins for medium-to-high volume (anything above ~50 calls/month per provider). Per-minute pricing rarely wins anywhere except for practices with very short, very predictable calls.

For a 3-provider practice with 200 calls/month/provider (~600 total), per-provider DeskMD at $897/mo is roughly half the cost of equivalent live human services. The trade is the AI on the line.
